Not everyone  can afford lotion moreover you can see that he's quite dark skinned and for other people it may be medication they are on that makes their skin so dry.

To me he seems to be someone who is going through a rough phase as most people do at some point in Africa. There comes a time when you really suffer and have no hope and having a meal a day is a struggle.
